Job Summary
Perform personal care tasks including bathing, oral hygiene, dressing, and feeding patients. Assist with ambulation, transfers, and range of motion exercises as directed. Monitor vital signs, observe skin integrity, and report significant changes to the Case Manager. Plan and prepare nutritious meals, maintain living areas, and provide respite for families. Adhere to documentation standards and participate in quality improvement teams. Work Monday through Friday from 8am to 5pm in Sun Prairie or De Forest.
Required Qualifications
- Must be a Certified Nursing Assistant
- Must have completed a minimum of 75 hours of classroom and supervised practical training
- Licensed driver with automobile that is insured in accordance with organization/state requirements and is in good working order
- Willingness to submit to a criminal background check prior to hire and annually thereafter
- Current CPR certification
Desired Qualifications
- Preferred to have one (1) year experience in community health/hospice or medical/surgical environments
- An Understanding of hospice philosophy and comfortable providing specialized care to the terminally ill
- Ability to work autonomously with little direct supervision
- Successfully demonstrate tact, patience and good personal hygiene
- Ability to communicate effectively
- Strong organizational skills
- Understanding of plan of care documentation
- Flexible in work hours to ensure the patients receive care as provided in the plan of care
- Ability to empathize with the needs of the ill, injured, frail and impaired
- Comfortable addressing issues of death/dying
- Calm demeanor when entering the patients home
