Certified Nursing Assistant for Hospice Services

Job Summary
Provide personal care services to hospice patients as prescribed by the physician and in compliance with state nurse practice act and applicable regulations. Deliver emotional and physical support to terminally ill individuals and their families, assist with activities of daily living, maintain patient safety and a clean environment, assist with light housekeeping and meal-related tasks, document services in the patient record, participate in interdisciplinary care planning, and support the patient’s comfort and dignity in the home setting. Collaborate with the RN and hospice team, and assist with medication reminders and routine care tasks as directed by the care plan.
Required Qualifications
- Completion of state recognized and federally compliant CNA training program and/or competency evaluation
- at least six-months experience as a Certified Nursing Assistant
