Care for seniors and those with illnesses or chronic diseases in their homes by assisting with daily activities like bathing, toileting, dressing, grooming, and nutritional intake. Support patient mobility through ambulation, transfers, and range of motion exercises while monitoring vitals, intake/output, and blood glucose levels. Collaborate with the team to ensure care plans are followed and notify supervisors of any issues requiring changes in medication or equipment. Maintain a 1:1 patient ratio and work independently to manage your own schedule. Requires active Nursing Assistant Certification in SC, CPR certification, and a valid driver's license.
