Job Summary
Provide hands-on patient care in a skilled nursing setting, including bathing, dressing, grooming, toileting, and feeding; assist with transfers, ambulation, and repositioning using safe lifting techniques; monitor and report changes in patient condition; accurately document patient care and daily activities; follow individualized care plans to improve patient outcomes; maintain a clean, safe, and infection-controlled environment; ensure medical equipment is functioning properly; participate in shift handoffs and ongoing training; follow all policies and procedures; promote patient dignity, independence, and safety.
Required Qualifications
- Active Certified Nursing Assistant (CNA) certification (required)
- Ability to lift, move, and transfer patients safely
- Strong teamwork, reliability, and patient-first mindset
- Ability to read, write, and speak the English language.
