Job Summary
Provide one-on-one home-based care to seniors and others with illness, injury or chronic disease in patients' homes. Responsibilities include assisting with daily activities (bathing, toileting, dressing, grooming, hygiene, nutrition), helping with ambulation, transfers and range of motion, checking vitals, documenting changes, and ensuring the patient’s care plan is followed. Work independently within a team, observe patient needs, and notify supervisor of changes. Requires high school diploma (or equivalent) and active SC CNA certification, CPR, valid SC driver’s license, and ability to lift up to 50 lbs. Required Qualifications
