Job Summary
Health Partner role within Daymark's Care Team: a CNA/Medical Assistant position coordinating social and logistical aspects of patient care for oncology patients via phone, video, and in-home visits. Responsibilities include conducting structured Social Determinants of Health assessments, connecting patients with community resources (transportation, financial, food, medical equipment, emotional support), developing care plans, participating in case conferences, ensuring virtual visit readiness, scheduling follow-ups and referrals, preparing and maintaining accurate patient charts in the EHR, tracking follow-ups, and documenting interventions. Requires 3+ years in clinical or care-coordination roles (CNA/Medical Assistant/EMT), comfort with Slack/Zoom/Google Suite/EHR, strong relational skills, ability to drive to patient locations, and willingness to work in Daymark's Providence, RI office (5 days/week) with in-home visits as needed. Bilingual is a plus. Daymark Health requires BLS and adherence to vaccination/testing policies; valid driver's license is required for in-home visits.
Required Qualifications
- Certified Nursing Assistant (CNA) or Medical Assistant/EMT with 3+ years experience
- BLS certification (Basic Life Support)
- Driver's license (valid)
- Proficiency with Slack, Zoom, Google Suite, and EHR systems
- Willingness to travel to patient homes
- Willingness to work from Daymark's office in Providence, Rhode Island 5 days/week
