Job Summary
Deliver compassionate, high-quality care to patients across the lifespan while partnering with providers to ensure seamless, coordinated treatment. Support patients and families in understanding health needs and next steps, documenting all care accurately using the EHR system. Participate in care huddles and team-based quality improvement initiatives as a key member of the Patient-Centered Health Home model. Required CNA certification, active NC license, and current CPR/BLS certification. One year of clinical experience in primary care, community health, or outpatient settings is preferred, along with familiarity with underserved populations and PCMH/PCHH models. Required Qualifications
- CNA certification, active and in good standing with the State of North Carolina
- HS Diploma/GED
- Current CPR/BLS certification
- Valid Driver's License

- Experience with underserved or rural populations
