Job Summary
Serve as the primary point of contact for patients after their intake visit, helping them understand and act on their care plan by scheduling appointments, finding in-network providers, and using Medicare benefits. Resolve navigation friction regarding portals, referrals, appointment logistics, forms, and provider communications while identifying practical and social-needs support like food, housing, and transportation to connect patients to available resources. Coordinate with clinical team members for non-clinical support, document every interaction accurately in the platform, and maintain confidentiality in compliance with HIPAA. This remote 1099 independent contractor role requires a minimum commitment of 20 hours per week, generally during core business hours (9:00 a.m.–5:00 p.m. ET, Monday–Friday), with a rate of $20–$25 per hour.
Required Qualifications
- Certified Nursing Assistant (CNA) certification / state registry in good standing; or equivalent hands-on patient-care and patient-navigation experience
- 2+ years of patient- or community-facing support experience (care coordination, case management, member advocacy, health coaching, benefits navigation, community health work, or similar)
- Working knowledge of the U.S. healthcare system and Medicare
- Comfort working independently, managing a caseload, and maintaining strong documentation
- Reliable computer, internet, and a quiet space for patient calls
- Commit at least 20 hours per week
- Opportunity to work up to 40 hours per week
- Generally during core business hours (9:00 a.m.–5:00 p.m. ET, Monday–Friday)
- Consistent weekday availability
Desired Qualifications
- Experience supporting older adults or patients with complex or serious illness
- Familiarity with Medicaid and other public programs
- Bilingual — Spanish, or another language common among Medicare patients
