Job Summary
Perform personal care needs including baths, oral hygiene, and bed linen changes as assigned. Provide patient and family with positive communication techniques while maintaining confidentiality and safety protocols. Assist with dressing, feeding, ambulation, and medication reminders under state regulations. Monitor skin condition to prevent tissue breakdown and report significant changes to the Case Manager. Take vital signs when ordered and maintain patient living areas clean and orderly. Participate in quality assessment teams and adhere to documentation standards. Provide respite for family caregivers when on-site. This role supports Bristol Hospice's family-centered approach to end-of-life care.
Required Qualifications
- Certified Nursing Assistant
- Minimum of 75 hours of classroom and supervised practical training
- Licensed driver with automobile that is insured in accordance with organization/state requirements and is in good working order
- Willingness to submit to a criminal background check prior to hire and annually thereafter
- Current CPR certification
Desired Qualifications
- One (1) year experience in community health/hospice or medical/surgical environments
- Ability to read and follow written instructions
- An Understanding of hospice philosophy and comfortable providing specialized care to the terminally ill
- Ability to work autonomously with little direct supervision
- Successfully demonstrate tact, patience and good personal hygiene
- Ability to communicate effectively
- Strong organizational skills
- Understanding of plan of care documentation
- Flexible in work hours to ensure the patients receive care as provided in the plan of care
- Ability to empathize with the needs of the ill, injured, frail and impaired
- Comfortable addressing issues of death/dying
- Calm demeanor when entering the patients home
