Certified Nursing Assistant- 2nd Shift

Provide personal care, bathing, grooming, hygiene assistance, and comfort measures in accordance with each patient's individualized plan of care. Observe, document, and promptly report changes in patient condition to the supervising nurse, and assist with mobility, positioning, and activities of daily living while maintaining a safe care environment. Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase). Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life. No nights, no weekends, and no on-call rotation required.
