Job Summary

Evaluate and manage patients with chronic kidney disease, ESRD, hypertension, and electrolyte disorders across outpatient clinics, dialysis units, and hospital settings. Develop proactive care plans to slow disease progression, optimize medication management, and support smoother transitions between care environments. Conduct dialysis rounds for in-center and home modalities while managing vascular access issues and educating patients on transplant options. Collaborate with nephrologists, dietitians, and social workers to coordinate care for patients with multiple combidities. Requires a Master's or Doctorate in Nursing, current New Mexico licensure, national board certification, and DEA license. Required Qualifications

  • Master's or Doctorate in Nursing (MSN or DNP)
  • Current licensure as a Nurse Practitioner in the State of New Mexico (or eligibility)
