Job Summary
Provide hands-on personal care and activities of daily living according to the aide care plan as directed by the Registered Nurse or Therapist. Complete routine and complex assignments for assigned home care and hospice patients within the allotted time, including documentation of care provided and communication with other disciplines. Observe changes in patient functional status and personal care needs, documenting observations and reporting findings to the supervising clinician or clinical manager. Build strong relationships with patients, family members, caregivers, and the interdisciplinary care team while managing schedules to meet patient needs. Requires ability to drive to patient homes, lift over 50 lbs, and work weekends and holidays.
Required Qualifications
- High School Diploma/GED
- 1 year Experience in a healthcare setting as a nursing assistant or nursing aide
- Knowledge of medical terminology
- Strong time management skills
- Ability to drive to patients homes, regardless of weather and traffic conditions, and manage schedule to meet patient needs
- Ability to work in patient homes
- Ability to work with and support terminally ill patients and their family members/caregivers
- American Heart Association Basic Life Support - State of Pennsylvania
- CNA - Certified Nursing Assistant - State of Pennsylvania
- Home Health/Hospice - must possess and maintain personal car insurance at PA State minimum requirements
- Lift over 50 lbs
- Push/pull up to 140 lbs
- Carry 80 lbs short distances
- Frequent standing/walking, squatting, kneeling, crawling, climbing and reaching overhead
- Weekend Shift
