Job Summary
Provide personal care services to home health and hospice patients, including bathing, grooming, dressing, and toileting; assist with mobility, transfers, and ambulation; monitor and report changes in patient condition to supervising nurse; assist with vital signs; support activities of daily living; maintain a clean, safe environment and follow care plans as directed by the RN/LVN; document care provided accurately and timely; comply with HIPAA and safety standards; require CNA certification with valid license in TX (or applicable state); strong communication, compassionate demeanor, and ability to work independently in patient homes.
Required Qualifications
- Certified Nurse Aide (CNA) license in the state of Texas (or applicable state)
- High School Diploma or GED preferred
- Current CPR certification (preferred or required depending on agency policy)
- Experience in Home Health, Hospice, Nursing Home, or Hospital setting preferred
- Familiarity with ADLs and patient care documentation
- Strong communication and interpersonal skills
- Ability to work independently in patient homes
- Reliable transportation and valid driver’s license (if required for field visits)
