Job Summary
Provide personal care services (bathing, grooming, dressing, toileting) and assistance with mobility/ADLs in home health and hospice settings under RN/LVN supervision. Monitor and report changes in patient condition, take vital signs as directed, document care accurately, follow care plans, maintain a clean and safe environment, and ensure HIPAA compliance. Requires CNA license in Texas, HS diploma/GED preferred, CPR may be required, and prior experience in home health, hospice, or related settings is preferred. Strong communication, compassion, reliability, and ability to work independently in patients' homes are essential.
Required Qualifications
- Must be a Certified Nurse Aide (CNA) with a valid CNA license in the state of Texas (or applicable state)
- High School Diploma or GED preferred
- Current CPR certification (preferred or required depending on agency policy)
- Experience in Home Health, Hospice, Nursing Home, or Hospital setting preferred
- Experience working with elderly, disabled, or terminally ill patients strongly preferred
- Familiarity with ADLs and patient care documentation
- Strong communication and interpersonal skills
- Reliable transportation and valid driver’s license (if required for field visits)
- Ability to work independently in patient homes
- Documentation accuracy
