Job Summary

Provide routine Activities of Daily Living (ADL) care and perform person-centered care according to resident care plans and facility standards. Communicate professionally with residents, families, and providers while documenting ADLs, reporting condition changes, and filing incident reports in electronic health records. Assist with admitting, transferring, and discharging residents as assigned by the charge nurse, maintaining safe and tidy environments with strict adherence to infection control practices. This role requires current C.N.A. certification, high school education, and the ability to lift up to 40 pounds. Applicants must have reliable transportation and basic computer skills, with two years of experience preferred. The position involves working in an assisted living environment with potential exposure to infectious conditions and fluctuating environmental factors.

Required Qualifications

  • High School
  • Completion of C.N.A. course (Certified Nurse Aide)
  • Current C.N.A certification (Certified Nurse Aide)
  • Ability to lift up to 40 pounds
  • Ability to ensure timely arrival and stable transportation
  • Ability to work in the environment conducive to caring for residents, with or without reasonable accommodation and without posing a direct threat to self or others
  • Ability to move throughout the buildings and grounds as needed throughout the day
  • Excellent communication skills
  • Basic computer skills
  • Ability to communicate professionally and effectively with residents, families, professional providers, and teammates
  • Ability to perform and accurately document ADLs in electronic record
  • Ability to document and verbally report resident conditions/changes in conditions in EHRs according to facility procedures
  • Ability to document and report all incidents and/or complaints according to facility procedures
  • Ability to promote resident independence, interacting with residents in a manner promoting mental and physical well-being
  • Ability to perform daily care per the resident's care plan/profile, observed needs, according to facility standards of care and within state regulation and facility policies
  • Ability to assist with admitting, transferring, and discharging residents as assigned by the charge nurse
  • Ability to maintain the resident's environment in a safe and tidy manner, including adherence to infection control practices
  • Ability to recognize and respond and make prompt and accurate judgments with regard to resident care needs
  • Ability to perform assigned tasks in accordance with facility policies and procedures
  • Ability to complete and maintain compliance with Relias Learning courses
  • Ability to establish and encourage an atmosphere of optimism, warmth, and interest in residents' activity preferences and needs
