Deliver resident personal care including hygiene, feeding, and mobility assistance while maintaining accurate intake, output, and vital sign documentation. Assist with activities of daily living, transport residents to appointments and social programs, and maintain unit cleanliness and linen standards. Monitor resident stability, respond promptly to call lights, and review care plans to adjust routines as needed. Maintain confidentiality and adhere to federal, state, and local regulations. Participate in shift change reports and complete required forms and logs.
