Job Summary
Conduct patient care duties as a member of the primary care team, supporting patients and families in self-management and behavior change. Ensure continuity and coordination of care within the medical home. Drive to locations where required. Refer to Clinical Affairs Policy 900.042 and 2001.016 for core functions related to medical assistant duties and health center drug dispensing.
