Job Summary
Manage overall patient care for Northern Montana Hospital clients, specifically those requiring prenatal and postnatal services. Perform population health management, care coordination, and clinical documentation while facilitating efficient resource use. Act as an advocate to minimize health delivery fragmentation and assist patients in linking to community resources. Provide patient and family-centered continuity of care during transitions and demonstrate conflict resolution skills with clients. Requires current Registered Medical Assistant certification, Basic Life Support training, and a valid driver's license. Full-time position with health, dental, and 403(b) benefits; starting pay varies based on experience.
Required Qualifications
- High school diploma or GED equivalent
- Current Registered Medical Assistant Certification/CCMA
- Valid driver's license with acceptable driving record for insurance purpose
Desired Qualifications
- Up to date training and certification in Basic Life Support issued by the American Heart Association
