Job Summary
Direct care and clinic coordination for senior participants in a PACE program; duties include taking vital signs, assisting during exams, administering medications and immunizations, performing lab tests, coordinating patient flow, documenting in EHR, and educating participants on medications and follow-up care. Acts as a key connector between participants, providers, nurses, and the interdisciplinary team to ensure smooth communication and care. Maintains infection-control and safety standards and contributes to morning huddles and quality improvement activities. Requires current CMA credential and is a full-time, first-shift role at the Warren, OH site with comprehensive benefits.
Required Qualifications
- Current Certified Medical Assistant (CMA) credential
- Experience working with older adults in long-term care, primary care, or PACE program is preferred
- Familiarity with EHR systems and strong record-keeping skills
