Job Summary
Provide patient care as directed on aide care plan in clients' homes with 1:1 care, observe and report changes in client condition to Supervising Clinician, assist with Activities of Daily Living, communicate supply needs to Health Care team, maintain professionalism, comply with HIPAA and agency policies, document observations in prescribed format, provide companionship and patient care as directed on the care plan, and help families understand instructions from medically trained staff. Flexible scheduling options include Full-Time and Per Diem; Focus Home Health supports a patient-centered, team-driven environment in San Jose and surrounding South Bay communities.
Required Qualifications
- Must have vehicle, valid current driver’s license and automobile insurance
- Minimum 1-year experience
- High School Graduate or equivalent
- Current certification as a Certified Home Health Aide by the State of California
- Current CPR card
- Documentation to verify work experience and legal right to work in the U.S.
- Report to work in a timely manner and adhere to call-in times for schedule changes
- Able to provide two verifiable references
