Job Summary
Provide in-home care to clients per the plan of care as a Certified Home Health Aide (HHA). Perform personal care, light housekeeping, meal preparation, and errands; communicate patient status and progress with the clinical team; accurately document care on activity sheets; and adhere to HIPAA and all regulatory requirements. Requires NYS driver's license and HHA certification or eligible pathways (CNA with required competency steps or equivalent home health training). 40 hours per week across day/evening shifts in Newark, NY.
Required Qualifications
- NYS Driver's License
- HHA Certification or eligibility via CNA plus competency requirements
- Home Health Aide training and competency evaluation or equivalent
- English literacy (read/write/speak) for care instructions
- Ability to follow care plans and documentation requirements
