Job Summary
Provide one-on-one home health care to patients in patients' homes, following a one-on-one care plan. Responsibilities include maintaining records of patient care, conditions, progress or problems; reporting observations to supervisor; assisting with all ADLs (Activities of Daily Living), meal preparation, medication reminders, extensive transfer support (bed, chair, mobility transitions), ambulation assistance (wheelchair), skilled services (wound care), Hoyer Lift experience required, and transporting/supporting clients to Senior Center. Strong verbal and written communication skills are needed; prefer schedule Monday through Friday (10am to 2pm); transportation provided.
Required Qualifications
- Certified Home Health Aide (CHA) or CNA certification/license required
