Job Summary
Provide compassionate personal care including bathing, grooming, dressing, and mobility assistance. Prepare simple, nutritious meals based on patients' dietary needs and maintain a clean, safe home environment. Document all care using Celltrak and report patient condition updates to RN Case Managers. Morning shifts run 6:00am-2:30pm or 7:00am-3:30pm; tuck-in shifts are 4:00pm-8:00pm. Flexible part-time, full-time, or per diem options are available with pay starting at $20 per hour. Active NJ CHHA certification, reliable transportation, and willingness to travel throughout Monmouth County are required.
Required Qualifications
- Active NJ CHHA certification in good standing
- Strong communication skills in English (written and verbal)
- Ability to lift, push, or pull up to 50 lbs
- Willingness to travel throughout Monmouth County
- Reliable transportation
