Job Summary
Follow written instructions for patient care as prepared by an RN, assisting with personal care including lifting and transferring patients to maintain hygiene and safety. Perform simple procedures such as ambulation, ROM, and exercise, and assist in administering medications under physician or nurse direction. Report changes in patient condition to the Supervisor or RN and maintain records of services and patient status. Establish working relationships with patients, families, and co-workers while participating in care conferences. Provide household services essential to home health care. Requires CNA-LTC and HHA certification, current CPR, ability to lift 100 lbs, valid driver's license, and reliable transportation. Schedule is 10 hours weekly, contingent on patient needs.
Required Qualifications
- Must be CNA- LTC and Home Health Aide (HHA) certified
- Current CPR certified
- Must have the ability to lift, move, & transfer 100 lbs with the physical capability to perform all job-related duties
- Valid drivers license
- Reliable transportation
- Current state minimum vehicle insurance coverage
- Willingness to enforce health and safety standards
- High school diploma or GED
- Successful completion of training and competency evaluation program or competency evaluation program or nurse aide training and competency evaluation program approved by the state without a contiguous 24 months lapse
