Job Summary
Provide personal care including baths, oral hygiene, and changing bed linens while assisting patients with dressing, toileting, feeding, and medication administration. Perform range of motion exercises as ordered, maintain patient living areas, and ensure safety needs are met using equipment properly. Report significant patient condition changes to the assigned RN Case Manager and complete a computer literacy assessment prior to scheduling an in-person interview.
Required Qualifications
- Valid CHHA CA Certification
- Current BLS/CPR Certification
- Valid CA Driver's License
- Excel at building rapport with patients and providing the highest quality of care
- Self-Directed
- Empathetic
- Critical thinker
- Outstanding interpersonal skills
- Organized
- Must be in the field visiting patients in their homes
- Must be available for occasional coverage outside of primary territory
- Must complete a computer literacy and competency assessment prior to scheduling an in-person interview
