Job Summary
Provide personal care including baths, oral hygiene, and changing bed linen under the direct supervision of an RN Case Manager. Assist patients with dressing, toileting, feeding, and medication self-administration while performing range of motion exercises and maintaining a clean living area. Report significant patient condition changes to the assigned nurse and ensure safe use of equipment. You will work full-time in the field visiting patients in Alameda County, completing a required computer literacy assessment prior to scheduling an interview. Valid CHHA CA Certification, BLS/CPR, and a driver's license are required.
Required Qualifications
- Valid CHHA CA Certification
- Current BLS/CPR Certification
- Valid CA Driver's License
- Excel at building rapport with patients and providing the highest quality of care
- Self-Directed

- Outstanding interpersonal skills
- Organized
- Must be in the field visiting patients in their homes
- Must be available for occasional coverage outside of primary territory
- Must complete a computer literacy and competency assessment prior to scheduling an in-person interview
