Job Summary
Assist patients with personal hygiene, bathing, transfers, and range of motion exercises while accurately recording vital signs and intake/output. Perform bowel/bladder care, catheter maintenance, and apply dressings or colostomy bags as instructed. Administer medications, prepare meals, and observe physical and emotional status to report changes promptly. Complete all documentation via Celltrack, maintain a clean environment, and participate in required education programs. Utilize a cell phone and GPS to move between patient homes throughout the service area. Adhere to attendance standards, dress code, and employment policies.
Required Qualifications
- High School diploma or equivalent
- Current NJ Board of Nursing Home Health Aide Certification
- Knowledge of skills required to provide excellent care in accordance with the role of Certified Home Health Aide (CHHA) in an in-home hospice environment
- Self-starter who can work independently under the direction of the Registered Nurse
- Excellent interpersonal skills for interacting with patients and families, nurses and employees, and patients and families via telephone and in person
- Ability to comprehend the spoken and written English language, to communicate with clarity both verbally and in writing
- Well organized to manage time, prepare care-related documentation
- Ability to utilize a cell phone and GPS to move about as assigned to various patient homes throughout the service area
Desired Qualifications
- Experience working as a CHHA in a hospice environment
