Job Summary
Conduct comprehensive personal care for patients, including bathing, hygiene, toileting, and mobility assistance using a Hoyer lift. Administer medications, perform catheter and colostomy care, and manage bowel and bladder training programs. Monitor vital signs, intake and output, and patient status to report changes to the Registered Nurse. Prepare meals, assist with feeding, and provide post-mortem care as instructed. Maintain accurate documentation via Celltrack and ensure a clean environment. Work independently in an in-home hospice setting, utilizing GPS to navigate patient homes while adhering to attendance standards and professional dress code.
Required Qualifications
- High School diploma or equivalent
- Current NJ Board of Nursing Home Health Aide Certification
- Knowledge of skills required to provide excellent care in accordance with the role of Certified Home Health Aide (CHHA) in an in-home hospice environment
- Self-starter who can work independently under the direction of the Registered Nurse
- Excellent interpersonal skills for interacting with patients and families, nurses and employees, and patients and families via telephone and in person
- Ability to comprehend the spoken and written English language, to communicate with clarity both verbally and in writing
- Well organized to manage time, prepare care-related documentation
- Ability to utilize a cell phone and GPS to move about as assigned to various patient homes throughout the service area
