Certified Home Health Aide (CHHA)-Community

Certified Home Health Aide (CHHA) provides comprehensive patient care in a home setting, including bathing and personal hygiene, vital signs, safe transfers to bed/chair/commode, range of motion exercises, I&O documentation, bowel/bladder care and incontinence management, catheter care, ostomy care, applying dressings, sitz baths, and assist with prescribed medications under RN supervision. Responsibilities also include meal preparation and feeding per dietary instructions, observing and reporting changes in patient condition, completing accurate documentation via Celltrack, assisting with post-mortem care as instructed, maintaining a clean patient environment, participating in required in-service education, and adhering to attendance and dress code policies.
