Job Summary
In-home Certified Home Health Aide to support patients in Santa Clara and San Benito counties. Provide compassionate, personalized care including activities of daily living (bathing, dressing, grooming), administer medications as prescribed, monitor vital signs, assist with mobility and transfers, offer companionship and emotional support, assist with meal planning/preparation and feeding, follow care plans, and maintain a clean, safe environment while ensuring patient confidentiality. Knowledge of hospice care, dementia care, and memory care is a plus; strong communication and teamwork skills are essential.
Required Qualifications
- Previous experience in home health or related field preferred
- Ability to administer medications accurately and safely
- Familiarity with HIPAA regulations and ability to maintain patient confidentiality
- Compassionate and empathetic nature, with a genuine desire to help others
