Job Summary
Provide or assist residents with activities of daily living, including personal hygiene, grooming, toileting, positioning, and transfers. Support prescribed exercise and range of motion activities, assist with light housekeeping such as changing bed linens, and help with feeding and meals. Monitor vital signs like temperature, pulse, and respirations as supervised, then report any condition changes to the Hospice or facility nurse while documenting reports on the certification form. Remind patients to take medication in compliance with regulatory standards. Requires a State Certified CHHA license, high school diploma, valid driver's license, and willingness to travel. Full-time, part-time, and per diem shifts available across multiple locations.

Desired Qualifications
- Experience in hospice or home health, preferred
- No experience necessary but 1 year, preferred
