Job Summary
Provide in-home patient care as a Certified Home Health Aide per the care plan; assist with activities of daily living (personal care, light housekeeping, meal preparation, and errands), and maintain effective communication with patients and supervision staff. Travel between sites in Rochester and Newark, NY with mileage reimbursement; typically 30 hours per week, days schedule. Requires active HHA certification or CNA/related training with competency steps within 30 days of hire; prior experience with frail elderly population preferred; must read, write, and follow instructions and adhere to HIPAA and departmental protocols.
Required Qualifications
- Active Home Health Aide (HHA) certification
- HHA with lapse of <= 2 years and completion of written exam and 1-day HHA Skills Competency Class within 30 days of hire OR Certified Nursing Assistant (CNA) willing to complete a written exam and 1-day HHA Skills Competency Class within 30 days of hire OR Out-of-state HHA or Nurse Aide training with documented competency evaluation OR Nursing student (Fundamentals of Nursing completed within past 24 months) OR U.S. Military-trained medic or medical technician
- Preferred Qualifications: High school diploma or equivalent
- 1 year experience working with the frail elderly population
- Compassionate, patient focused and dependable professional
- Critical thinking skills, decisive judgment and ability to work with minimal supervision
- Exceptional documentation and communication skills
- Ability to follow verbal and written instructions
