Job Summary

Verify the Plan of Care prior to each visit and provide care according to delegation by the RN, physician's orders, and established standards. Incorporate patient care goals into nursing visits as evidenced by documentation on skilled nursing notes. Communicate significant findings, problems, and changes in condition to the RN Case Manager, physician, and clinical supervisor. Report unsafe conditions and implement patient safety plans using community resources. Assist patients in learning self-care techniques and suggest revisions to the plan of care based on identified issues. Document the medication regimen and update the medication profile according to established procedures. Treat patients and caregivers effectively while implementing actions to prevent avoidable hospitalizations and ER visits. Provide positive, supportive communication to physicians, patients, families, and agency personnel. Collaborate with the RN Case Manager during normal business hours for advice and instructions.

Required Qualifications

  • High School Diploma or GED equivalent
  • 1 year of experience as a Hospice Aide or Nursing Assistant
  • State-established or other Hospice Aide training program that meets the requirements of 42 CFR §484.36 and a competency evaluation program
  • 50% travel
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Valid driver's license and insurance and reliable transportation to perform job tasks
  • EVerify participation after a job offer is accepted and Form I-9 completed
