Job Summary
Coordinate care for patients with diabetes through specialized teaching, consultation, and guidance of the health care team while maintaining certification and completing one formal presentation annually. Deliver clinical services guided by organizational Standards of Patient Care and Nursing Practice, ensuring continuous improvement in patient outcomes. Maintain minimum of 18 continuing education credits per year and work within an ADA Recognized Education Program. Located in Burlington, Vermont, the team offers structured preceptorships, shared governance, and a culture committed to great patient ratios.
Required Qualifications
- Current RN licensure or compact licensure recognized by the State of Vermont
- ADN
- Minimum of 3 years relevant clinical experience in diabetes
- Incumbent must work in an ADA Recognized Education Program or other nationally recognized diabetes education program
- BLS Certification
- Certified Diabetes Care and Education Specialist certification current
- Minimum of 18 continuing education hours/year
Desired Qualifications
- BSN
- Current internal RNs pursuing a BSN to support the initiative
