Certified Diabetes Educator

Job Summary

Deliver prediabetes, diabetes, and prevention education programs across Napanee and surrounding communities, managing insulin initiation and home visits as needed. Collaborate with partner agencies, hospitals, and primary care providers to facilitate group sessions, screen clients, and provide referrals while maintaining up-to-date electronic health records. Organize outreach events, coordinate with reception staff for phone screenings, and adapt resource packages to meet diverse client needs. Work within an interdisciplinary team to ensure comprehensive care, monitor program targets, and participate in chart reviews and case conferences.

Required Qualifications

  • Undergraduate degree in foods and nutrition from a recognized university
  • Current registration as a Registered Dietitian with the College of Dietitians of Ontario
  • Demonstrated experience working with people who have diabetes
  • A minimum of 3 years of relevant community nutrition experience
  • Valid Driver's License, valid insurance coverage, and access to vehicle
  • Current and satisfactory Criminal and Vulnerable Persons Check
  • Certified Diabetes Educators (CDE) certification or in progress

Desired Qualifications

  • Experience working in rural areas, with Indigenous health issues, and seniors
  • Experience in program development, implementation, monitoring and evaluation
  • French or Indigenous language
