Job Summary
Provide individual and group education and support to patients and families regarding diabetes and other chronic health conditions. Deliver high-quality, compassionate healthcare while promoting health and wellness within the community. Maintain certification as a Certified Diabetes Care and Education Specialist and hold required state licensure. Complete Basic Life Support training within 90 days of hire. Requires a registered nurse, dietitian, social worker, or exercise physiologist with at least two years of relevant patient care experience.
Required Qualifications
- RN
- Dietitian – BS
- Social Work – MSW
- Exercise Physiologist -MS
- Two years experience in nursing, dietetics, social work, or exercise physiology, including patients with diabetes
- Certified Diabetes Care and Education Specialist
- licensure required by State of Alabama dependent on professional designation
- BLS within 90 days of DOH/Transfer
Desired Qualifications
- Advanced degree
- Two years experience in diabetes patient care management and education
