Job Summary
Collaborate with physicians, Health Services Coordinators, and nursing staff to identify patient populations requiring intensive diabetes education programs. Develop custom patient education materials by working with Marketing, Health Promotion, and Patient Services. Provide direct teaching and management of high-risk diabetic patients using established endocrinologist algorithms. Establish, maintain, and monitor diabetes education programs while ensuring clinic compliance with regulatory and governmental standards. Function as a Case Manager for Diabetes patients, select educational vendors, and address infection control data to stabilize complications. Review multidisciplinary team functioning, resource availability, and patient expectations to ensure safe, efficient care delivery. Attend Diabetes Committee meetings as assigned.
Required Qualifications
- RN
- CDE and RN
- 2+ years of related clinical experience, patient education, program development
- Proven excellent Written and verbal communication skills
- Proven excellent organizational skills
