Job Summary
Certified Community Caregiver with driving responsibilities providing safe, courteous, on-schedule transportation to residents and personalized daily living assistance (bathing, dressing, grooming, meal prep). Administers medications and monitors health conditions, reports changes to healthcare professionals, and offers companionship and emotional support while maintaining accurate care records and coordinating with family members and healthcare teams. Requires valid AZ caregiver certification and multiple clearances/certifications (TB clearance, Fingerprint Clearance, First Aid/CPR, Food Handlers, Memory Care), background/credit/motor vehicle screenings, and a valid AZ driver’s license. Location: Phoenix, AZ; role emphasizes independence, dignity, and safe, compassionate care.
Required Qualifications
- Valid certification as a Caregiver in the state of Arizona
- High school diploma or equivalent education
- Must show proof of freedom from TB before client contact
- Provide and maintain a current Level One Fingerprint Clearance Card
- Provide and maintain a current certification in First Aid and CPR
- Provide and maintain current Food Handlers Certification
- Provide and maintain a Memory Care Certification
- Clear a criminal background, credit check, and motor vehicle screening
- Clear the Department of Economic Security, Department of Aging and Adult Services Central Registry Background Check
- Maintain a valid AZ driver's license, a clean driving record, and reliable transportation throughout the duration of employment
