Central Discharge Planner

Coordinate quality, cost-effective patient care by linking the multi-disciplinary patient care team with Skilled Nursing Care facilities, Board and Care facilities, and patient transportation resources. Manage discharge planning to ensure seamless transitions for patients leaving the hospital system. This full-time days shift role operates within the Case Management department.
