Job Summary
Lead the clinical team at your center by practicing primary care at 80% clinical time while managing 20% administrative duties. Coordinate provider onboarding, model evidence-based care, and ensure robust implementation of daily huddles, patient care planning, and admission prevention workflows. Cascade organizational strategy from regional leadership, address root causes when metric goals are missed, and conduct performance reviews to coach providers toward excellence. Maintain a culture of growth through constructive feedback, celebrate team successes, and support the value-based full risk model of care. This full-time role requires an M.D., D.O., or A.P.N./P.A. with board certification and 5+ years in outpatient practice, offering a base salary between $174,070 and $374,920.
Required Qualifications
- M.D., D.O., or A.P.N or P.A. graduates
- Board certification in Internal Medicine or Family Medicine required for M.D. or D.O.
- Board certification in Family, Adult, or Gerontology for A.P.N.
- Experience in clinical leadership roles, leading and coaching providers to be the best they can be for their patients and their colleagues
- Experience using a metrics-driven approach to the provision of medical care and/or quality projects
- 5+ years of experience in outpatient practice preferred
- US work authorization

- Fellowship training in Geriatrics
- Other professional degrees (e.g., M.B.A., J.D., M.P.H.)
