Job Summary
Lead the clinical team at your center by practicing primary care at 80% clinical time while managing 20% administrative duties. Collaborate with Practice and Outreach Managers to drive organizational strategy, implement care models, and ensure evidence-based admission prevention workflows. Coordinate provider onboarding, conduct performance reviews, and mentor colleagues to achieve triad best practices and center metric goals. Cascade organizational information to care teams and escalate concerns to regional leadership. Maintain a culture of coaching and joy in practice while supporting the full risk model of care.
Required Qualifications
- M.D., D.O., or A.P.N or P.A. graduates
- Board certification in Internal Medicine or Family Medicine required for M.D. or D.O.
- Board certification in Family, Adult, or Gerontology for A.P.N.
- Experience in clinical leadership roles, leading and coaching providers to be the best they can be for their patients and their colleagues
- Experience using a metrics-driven approach to the provision of medical care and/or quality projects
- US work authorization
- 5+ years of experience in outpatient practice preferred
Desired Qualifications
- Fellowship training in Geriatrics
- Other professional degrees (e.g., M.B.A., J.D., M.P.H.)
- Prior executive roles
