Job Summary
Prepare Care Plans and Team Care Arrangements to help patients manage disease, while conducting health assessments for early chronic disease identification. Coach patients on embedding health behaviour change principles and empower them to increase ownership over their health. Enable patients to access ongoing care from a multi-disciplinary team and work with GPs to promote preventative healthcare throughout the practice. Ensure patient care plans are reviewed regularly and manage administrative tasks relating to CDM appointments. This role supports GPs and local communities in preventative health within the ForHealth Medical Centres network.
Required Qualifications
- AHPRA registered Enrolled or Registered Nurse
- Proven prior experience in Chronic Disease Management and health assessments
Desired Qualifications
- Knowledge of Health Assessment is highly regarded
- Strong organisational and time management skills
- Outstanding communication and customer service skills
