Job Summary
Prepare GP Management Plans and Team Care Arrangements to support patients managing chronic conditions. Conduct patient-centred health assessments for early disease identification and prevention. Work with patients to establish SMART health goals and coach them toward sustainable behaviours. Coordinate holistic care in partnership with GPs and connect patients with allied health professionals. Identify eligible patients for care planning, maintain accurate clinical documentation, and ensure compliance with Medicare requirements. Provide ongoing monitoring and regular care plan reviews. Join the Port Macquarie Medical Centre team on a permanent full-time basis with no nights, shift work, or weekends. Required Qualifications
- Current AHPRA registration as a Registered Nurse or Enrolled Nurse
- Previous experience in Chronic Disease Management, care planning or health assessments
- Knowledge of Medicare requirements relating to care plans and health assessments
- Strong organisational and time-management skills
- Excellent written and verbal communication skills
- The ability to motivate patients and encourage them to take ownership of their health
- A proactive, adaptable and team-focused approach
- A warm, personable manner and commitment to delivering an excellent patient experience
Desired Qualifications
- Previous Chronic Disease Management experience is highly regarded
- Experience within a general practice or primary healthcare environment is highly regarded
