Job Summary
Prepare GP Management Plans and conduct patient-centred health assessments to identify risks of chronic disease. Coach patients on sustainable health behaviours, establish SMART goals, and connect them with allied health professionals. Coordinate holistic care with GPs, maintain accurate clinical documentation, and ensure compliance with Medicare requirements. Identify eligible patients for first-time care planning and support team members in delivering preventative services. This permanent full-time role in Port Macquarie offers no nights, shifts, or weekends, with performance incentives and access to dental discounts and private health insurance.
Required Qualifications
- Current AHPRA registration as a Registered Nurse or Enrolled Nurse
- Previous experience in Chronic Disease Management, care planning or health assessments
- Knowledge of Medicare requirements relating to care plans and health assessments
- Strong organisational and time-management skills
- Excellent written and verbal communication skills
- The ability to motivate patients and encourage them to take ownership of their health
- A proactive, adaptable and team-focused approach
- A warm, personable manner and commitment to delivering an excellent patient experience
Desired Qualifications
- Previous Chronic Disease Management experience is highly regarded
- Experience within a general practice or primary healthcare environment is highly regarded
