- no more than 2 moving violations in last 2 years
- no serious traffic violations in last 12 months
- no more than 1 on-road or DOT preventable loss in last 2 years
- no DUI/DWI within past 10 years
- no SAP drivers on this account
- no unemployment over 90 days
- accident reports required on all accidents (last 5 years)
