CCS Service Facilitator

Job Summary
Conduct mental health assessments, including suicidal and homicide risk evaluations, and develop personalized recovery treatment plans for individuals with high-level mental health and substance use needs. Provide psycho-social rehabilitative services in diverse settings such as homes, schools, and community spaces to promote self-determination and empowerment. Facilitate individual and group psychoeducation, skill development, and trauma-informed support tailored to marginalized survivors, including clients of color, LGBTQIA+ individuals, and survivors of sexual trauma. Maintain accurate client records, case notes, and master lists in compliance with agency policies and RCC Helpline training requirements. Participate in weekly case reviews, supervision, and team meetings to ensure alignment with best practices and continuous improvement in service delivery.
Required Qualifications
- Bachelor's or master's degree in Social Work, Counseling, Psychology, or a related field
- Demonstrated experience providing trauma-informed, empowerment-based services to individuals with mental health and/or substance use disorders
- Clear understanding of privilege, oppression, and systemic barriers affecting client needs
- Commitment to cultural humility and trauma-informed care
- Excellent verbal and written communication skills
- Ability to document services clearly and professionally
- Ability to work flexible hours, including evenings or weekends, as needed to meet client needs
- Satisfactory criminal background check
- Commitment to maintaining client confidentiality and adhering to ethical standards in all interactions
Desired Qualifications
- Bilingual (Spanish/English) proficiency
- Knowledge of Dane County community resources and local support systems
- Personal or professional experience in communities of color or other marginalized communities
