Job Summary
Responsible for the full client care cycle focused on assessing, planning, implementing, monitoring, and evaluating actions to meet clients' Health and Human Services needs in a case management role. Requires coordination of care, crisis intervention, advocacy, and compliance with clinical documentation standards.
Required Qualifications
- Ability to relate to and interact with a wide variety of people

- Ability to utilize technology including electronic service documentation systems
Desired Qualifications
- Bachelor’s degree in behavioral science or related field
