Job Summary
The CCBHC Case Manager is responsible for addressing Social Determinants of Health (SDOH) needs, assisting individuals in attaining stability across life domains, and providing referrals and advocacy. This role conducts intake assessments for person-centered treatment plans, collaborates with a multi-disciplinary team, and utilizes Motivational Interviewing and brief counseling techniques. Required qualifications include a High School Diploma or GED and relevant experience or a Bachelor’s Degree. Skills in communication and cultural sensitivity are essential.
Required Qualifications
- High School Diploma or GED required
- 1-3 years of relevant work experience in a behavioral health setting OR a Bachelor’s Degree
- Must complete required training(s) within three (3) months of hire date
- Valid NYS Driver's license
