Job Summary
Provides physical therapy and education to clients in a home-care setting; conducts assessments, initiates planning and oversight of care, and makes interdisciplinary referrals; may case manage clients across assigned geographic regions; coordinates education for clients and families, and collaborates with a broader care team to implement treatment plans and goals; requires active Physical Therapist licensure in MN, basic life support, and a valid driver's license. 8-hour day shifts, casual (0.0 FTE) with occasional weekends.
Required Qualifications
- Bachelor's degree in Physical Therapy from an APTA accredited program
- Licensed Physical Therapist - MN State Board of Physical Therapy required if providing services in MN
- BLS Tier 1 - Basic Life Support - Multisource required by completion of orientation
- Valid Driver’s License required upon hire - MN and WI residents must obtain a valid driver’s license in their state of residence within 60 days of employment
