Job Summary

Coordinate mental health and supportive services across healthcare providers, social service agencies, schools, housing programs, and other community partners. Establish measurable goals aligned with client strengths, needs, and recovery objectives. Track client progress and adjust services as needed. Ensure all services are documented to support clinical necessity and billing compliance. Support clients in navigating complex systems and achieving greater independence and stability. This role functions within a community mental health model where therapy is brought into everyday settings like neighborhoods, schools, and support groups. Flexible scheduling is required to accommodate client needs.

Required Qualifications

  • Illinois standards as a Mental Health Professional (MHP) or Qualified Mental Health Professional (QMHP)
  • Bachelor's or Master's degree in Social Work, Psychology, Counseling, or a related behavioral health field
  • experience in community-based case management or care coordination
  • knowledge of community resources in the Chicagoland area
  • experience working with a homeless or high-risk of homelessness population
  • experience working with individuals with serious mental illness (SMI), co-occurring disorders, or high-acuity behavioral health needs
